We are seeking an experienced Registered Nurse with strong OASIS skills for a 13-week full-time home health contract. Schedule & Contract Details:
- 13-week duration with potential for extension
- Full-time, Monday through Friday schedule
- On-call responsibilities may be required
- Consistent daily visit expectations
Key Responsibilities:
- Deliver in-home skilled nursing care, including wound care, patient education, medication management, and disease monitoring
- Conduct patient assessments and ensure timely, accurate completion of OASIS documentation
- Collaborate closely with physicians and interdisciplinary care teams to develop and implement individualized care plans
- Communicate changes in patient condition or care needs promptly to the clinical team
- Maintain clear, professional records and support quality patient outcomes
Qualifications:
- Active RN license (required)
- At least one year of recent home health RN experience
- Demonstrated proficiency with OASIS documentation (required)
- Experience with HomeCare HomeBase is strongly preferred
- Reliable transportation and a valid drivers license
- Ability to maintain a structured, full-time schedule with daily visit expectations
