A rewarding opportunity is available for a skilled Registered Nurse (RN) with home health and OASIS experience to provide personalized nursing care in a community setting. This full-time, 13-week nursing contract offers a structured schedule with Monday through Friday hours, allowing you to leverage your clinical expertise and documentation skills to positively impact patients lives.
Key Responsibilities:
- Deliver in-home nursing care including skilled interventions, wound care, medication management, and patient education
- Perform thorough patient assessments and complete OASIS documentation accurately and promptly
- Collaborate with physicians and multidisciplinary care teams to develop and implement individualized care plans
- Monitor patient conditions and communicate clinical updates to the care team as needed
- Manage daily patient visits according to a full-time schedule
Desired Qualifications:
- Active RN license with a minimum of one year recent home health nursing experience
- Demonstrated proficiency in OASIS documentation is required
- Experience with HomeCare HomeBase software is strongly preferred
- Ability to reliably manage daily visit schedules with dependable transportation and a valid drivers license
