We are searching for a dedicated Registered Nurse (RN) with strong OASIS experience to support a full-time, 13-week home health contract. This role is perfect for professionals confident in their clinical abilities, documentation, and time management, ready to excel in a structured, community-based setting.
Schedule:
- Full-time, Monday through Friday
- On-call requirements likely
- Daily structured patient visit expectations
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- OASIS assessment and documentation experience (required)
- Proficiency with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
Duties and Responsibilities:
- Deliver professional, compassionate in-home nursing care and skilled interventions
- Accurately perform patient assessments and complete OASIS documentation in a timely manner
- Work closely with physicians and the care team to implement individualized care plans
- Manage wound care, provide medication administration, patient/family education, and disease monitoring
- Clearly communicate any changes in patient condition to the clinical team
