This 13-week full-time contract position provides a chance to deliver compassionate, in-home patient care while utilizing your clinical skills and knowledge to the fullest.

Schedule/Hours:
- Full-time, Monday through Friday
- Potential on-call responsibilities
Key Responsibilities:
- Deliver skilled nursing care and interventions within patient homes
- Conduct thorough patient assessments and complete OASIS documentation accurately and promptly
- Collaborate closely with physicians and interdisciplinary care teams to develop individualized care plans
- Manage wound care, medication administration, patient education, and chronic disease monitoring
- Communicate clinical updates effectively to the healthcare team
Qualifications and Desired Experience:
- Current, active RN license
- Minimum of 1 year recent experience in home health nursing with OASIS proficiency
- Familiarity with HomeCare HomeBase is highly preferred
- Ability to maintain a structured schedule with daily visits
- Reliable transportation and valid drivers license
