An opportunity is available for a Registered Nurse (RN) with expertise in home health nursing on a contract basis in Bardstown, KY. This role focuses on delivering skilled patient care in home settings, emphasizing OASIS documentation and patient-centered treatment.
Key Responsibilities:
- Provide comprehensive nursing care to patients in their homes.
- Conduct initial and ongoing patient assessments including vital signs, medication management, and wound care.
- Develop and implement individualized care plans tailored to patient needs.
- Educate patients and families on disease management, treatment plans, and preventive care.
- Collaborate with physicians, therapists, and healthcare professionals to coordinate care.
- Accurately document patient care activities in electronic medical records (EMR).
- Monitor patient condition changes and report to supervising physicians.
- Ensure adherence to all state and federal home health care regulations.
Qualifications:
- Active RN license.
- Minimum of 3 months home health experience utilizing OASIS.
- Experience with HCHB is advantageous.
- Strong clinical skills in IV therapy, wound care, and medication management.
- Proficiency with EMR systems.
- Excellent communication and interpersonal abilities.
- Ability to work independently and manage time efficiently.
- Valid driver's license and reliable transportation.
