An exciting opportunity is available for a skilled Registered Nurse (RN) with a strong background in home health. This contract position is ideal for a compassionate nurse who thrives in independent patient care settings and is motivated to deliver top-quality outcomes within patients homes.
Key Responsibilities:
- Deliver comprehensive nursing care to individuals in their residences
- Conduct initial and ongoing assessments, covering vital signs, medication management, and wound care
- Develop and tailor care plans to meet unique patient needs
- Educate patients and families on disease processes, treatment protocols, and prevention strategies
- Collaborate closely with physicians, therapists, and interdisciplinary team members to coordinate care
- Accurately document care activities and findings in electronic medical records (EMR)
- Monitor patient status, reporting significant changes to the supervising physician
- Maintain strict adherence to federal and state home health regulations
Schedule:
- Contract role with potential for extension dependent on performance
Qualifications:
- Current and valid RN license
- Minimum 3 months of recent experience in home health utilizing OASIS documentation (required)
- HCHB (Homecare Homebase) experience preferred
- Superior clinical skills in IV therapy, wound care, and medication administration
- Proficiency in EMR systems
- Exceptional verbal and written communication abilities
- Capable of working autonomously and prioritizing assignments
- Active drivers license and dependable transportation
