Schedule:
- Full-time, Monday through Friday
- On-call responsibility is probable
- Structured daily visit expectations
Key Responsibilities:
- Deliver in-home nursing care, skilled interventions, and patient education
- Complete patient assessments and OASIS documentation with accuracy and timeliness
- Collaborate closely with physicians and interdisciplinary care teams to execute individualized plans of care
- Manage wound care, medication administration, and disease monitoring
- Communicate changes in condition and updates to the clinical team efficiently
Qualifications:
- Current, active RN license
- At least 1 year of recent home health RN experience
- Proven OASIS expertise is required
- Proficiency with HomeCare HomeBase is strongly preferred
- Ability to maintain a structured, high-volume visit schedule
- Reliable transportation and valid drivers license
