A healthcare role is available for a Registered Nurse (RN) specializing in home health nursing on a contract basis in Aurora, Illinois. Key Responsibilities:
- Deliver comprehensive nursing care to patients in their homes.

- Develop and implement individualized care plans tailored to patient needs.

- Collaborate with physicians, therapists, and other healthcare providers to maintain cohesive care.
- Accurately document all patient care activities promptly in electronic medical records (EMR).
- Monitor patient condition changes and report to supervising physicians.
- Ensure compliance with home health care regulations.
Qualifications:
- Active Registered Nurse (RN) license.
- Minimum of 3 months home health experience with proficiency in OASIS.
- Experience with Homecare Homebase (HCHB) software is advantageous.
- Clinical skills including IV therapy, wound care, and medication management.
- Proficient in electronic medical records systems.
- Strong communication and interpersonal abilities.
- Ability to work independently and manage time efficiently.
- Valid driver's license and reliable transportation.
