Job Description
An opportunity is available for an experienced Registered Nurse (RN) skilled in home health nursing to provide compassionate and comprehensive care to patients in their residences. This contract position requires a strong background in patient-centered care, particularly experience with OASIS assessments, to support individualized treatment plans in a home health setting.
Key Responsibilities:
- Perform initial and ongoing patient assessments, including vital signs, medication management, and wound care.
- Develop and implement individualized care plans based on patient needs.
- Educate patients and families on disease management, treatment plans, and preventive care.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care.
- Document patient care activities accurately and timely in electronic medical records (EMR).
- Monitor and report changes in patient conditions to supervising physicians.
- Maintain compliance with state and federal regulations related to home health care.
Qualifications and Experience:
- Active Registered Nurse (RN) license.
- Minimum of three months of home health experience with OASIS assessments.
- Strong clinical skills, including IV therapy, wound care, and medication management.
- Proficiency with electronic medical record systems; experience with HCHB is a plus.
- Excellent communication and interpersonal skills.
- Ability to work independently while managing time effectively.
- Valid driver's license and reliable transportation for patient visits.
