This 13-week contract provides the chance to deliver patient-centered care while collaborating within a supportive, professional care environment. Schedule Details:
- 13-week contract duration, with potential opportunities to extend
- Full-time, Monday through Friday
- Daily visit expectations
- Occasional on-call responsibilities
Key Responsibilities:
- Deliver skilled, in-home nursing care and interventions
- Complete initial and ongoing patient assessments with timely, accurate OASIS documentation
- Collaborate closely with physicians and multidisciplinary teams to execute individualized care plans
- Perform wound care, medication administration, patient/family education, and chronic disease monitoring
- Report condition changes promptly, maintaining clear communication with the clinical team
Role Requirements:
- Active RN license in good standing
- At least 1 year of recent home health RN experience
- Demonstrated OASIS documentation proficiency
- Strong preference for HomeCare HomeBase experience
- Reliable transportation and a valid drivers license
