This contract position focuses on providing exceptional nursing care in both home health and hospice settings. You will offer guidance, comfort, and expert clinical support for individuals navigating the end-of-life journey, ensuring dignity and compassion throughout their care.
Key Requirements:
- Active, unrestricted RN license
- Minimum 2 years of experience in hospice or palliative care
- Familiarity with home health clinical workflows and protocols
- Proficiency with Homecare Homebase (HCHB) preferred
- Excellent clinical assessment and communication abilities
- Compassionate, empathetic approach to each patient and family
- Valid drivers license and dependable transportation
Primary Responsibilities:
- Deliver skilled nursing care as part of a multidisciplinary team, focusing on physical, emotional, and psychosocial needs
- Conduct comprehensive assessments and coordinate individualized care plans
- Provide education and emotional support to patients and family members
- Monitor patient progress, administer medications, and communicate updates to physicians
- Document care and compliance using agency systems and HCHB (if applicable)
- Promote patient comfort and uphold quality of life at all times
