Job Description
This role offers the opportunity to deliver comprehensive, patient-centered care within nursing home and assisted living settings. The focus is on providing high-quality medical assessments and treatments to residents in skilled nursing and assisted living facilities, promoting in-place care to avoid unnecessary hospitalizations and assist patients in making successful transitions back to home environments.
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for each resident.
- Provide primary, preventative, urgent, and acute care on-site with hands-on bedside evaluations.
- Identify and stratify high-risk patients to ensure more frequent clinical visits when required.
- Develop contingency plans for managing high-risk conditions.
- Collaborate and communicate closely with residents primary care physicians and facility staff, including Directors of Nursing.
- Conduct single visits for patients transitioning from hospitals to the facility as part of discharge planning.
- Manage a panel of members across multiple nursing facilities, typically scheduled at one facility per day, with flexibility to cover other locations for acute patient assessments.
Qualifications:
- Master of Science in Nursing (MSN) degree.
- Active New York Nurse Practitioner license.
- Basic Life Support (BLS) certification.
- DEA registration.
- Experience in Long Term Care preferred.
Schedule:
- Monday through Friday, 8 AM to 5 PM.
- No weekends or holidays.
This contract position allows a focused work schedule with meaningful clinical responsibility in a specialized care environment.
