Job Description

A skilled Nurse Practitioner (NP) opportunity is available to deliver specialized care in nursing home and assisted living settings. This contract role centers on providing comprehensive clinical services to insured members residing in skilled nursing and assisted living facilities, aiming to manage care in place, reduce hospitalizations, and support seamless transitions back to home environments. Monthly health assessments, with increased frequency as needed, are critical to advancing patient health outcomes and avoiding emergency room visits when preventable.

Schedule:

  • Monday to Friday, 8 AM 5 PM
  • No weekends or holidays

Responsibilities:

  • Develop individualized problem lists and medical treatment plans for each resident
  • Deliver primary, preventive, urgent, and acute care on-site with hands-on bedside involvement
  • Identify and stratify high-risk patients to prioritize more frequent clinical visits and create contingency plans for these conditions
  • Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing
  • Conduct follow-up visits for patients newly transitioned from hospitals, ensuring safe rehabilitation within the facility
  • Manage care panels typically across multiple skilled nursing or assisted living facilities, with scheduling focused on one facility per day and flexibility to assess acute cases at other sites

Qualifications:

  • Master of Science in Nursing (MSN)
  • Current New York State Nurse Practitioner License and DEA certification
  • Basic Life Support (BLS) certification
  • Experience in Long Term Care is preferred
