A nursing professional opportunity is available to provide comprehensive care focused on patients in nursing home and assisted living environments. This role involves delivering high-quality primary, preventative, and urgent care to help minimize unnecessary hospitalizations and facilitate safe transitions back to home settings. You will have the chance to work closely with residents, facility staff, and primary care physicians to create effective care plans tailored to each patients needs.
Location
This position is based in Saratoga Springs, NY, a vibrant community known for its rich history, beautiful natural surroundings, and thriving arts scene. Schedule
- Monday through Friday
- 8 AM to 5 PM
- No weekends or holidays
Responsibilities
- Develop individualized problem lists and medical treatment plans for residents
- Provide hands-on bedside care and routine patient assessments in nursing and assisted living facilities
- Perform monthly health assessments, with increased frequency for high-risk patients
- Stratify high-risk patients for more frequent clinical visits and create contingency plans
- Collaborate closely with primary care physicians and facility staff, including Directors of Nursing
- Conduct single visits to patients transitioning from hospitals as part of discharge planning
- Manage a panel of members across multiple nursing facilities, typically seeing one facility daily, with flexibility to cover others as needed
Qualifications
- Master of Science in Nursing (MSN)
- Current New York State Nurse Practitioner license
- DEA certification
- Basic Life Support (BLS) certification
- Experience in Long Term Care preferred
