Job Description

This position offers a rewarding opportunity for an experienced Nurse Practitioner specializing in long-term care to provide comprehensive healthcare services within nursing home and assisted living environments. The role focuses on delivering high-quality, patient-centered care designed to reduce unnecessary hospitalizations and support effective transitions back to home settings.

Key Responsibilities:

  • Develop individualized problem lists and medical treatment plans for each resident.
  • Provide on-site primary, preventative, urgent, or acute care.
  • Identify and stratify high-risk patients to enable increased clinical oversight.
  • Create contingency plans tailored to high-risk conditions.
  • Collaborate closely with residents primary care physicians and facility staff.
  • Conduct routine bedside care, develop care plans, and create orders.
  • Perform assessments for patients recently transitioned to the facility from hospitals, facilitating safe discharge and care transitions.
  • Manage a panel of patients across multiple nursing facilities, with scheduling typically at one facility per day and flexibility to visit others as needed.

Qualifications:

  • Master of Science in Nursing (MSN) degree.
  • Current Nurse Practitioner license in New Hampshire.
  • Basic Life Support (BLS) certification.
  • DEA registration.
  • Experience in Long Term Care is preferred.

Schedule:

  • Monday to Friday, 8 AM to 5 PM.
  • No weekends or holidays required.
