Job Description
Deliver advanced clinical care to patients in skilled nursing and assisted living settings with a focus on reducing hospitalizations and facilitating smooth transitions back to home environments. This role offers the opportunity to provide comprehensive, hands-on care tailored to high-risk patients who need frequent assessments and individualized treatment plans.
Responsibilities:
- Conduct thorough health assessments on a minimum monthly basis, increasing frequency as needed to address acute issues and prevent emergency room visits or hospital admissions
- Develop and maintain individualized problem lists and medical treatment plans for each resident
- Provide primary, preventative, urgent, and acute care directly at the facility
- Identify high-risk patients and implement more frequent clinical visits alongside contingency plans for managing complex conditions
- Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing, to ensure coordinated care
- Perform bedside care and create detailed care plans and medical orders
- Evaluate patients transitioning from hospital discharge to ensure safe and effective recovery within the nursing or assisted living facility
- Manage panels of members across multiple facilities, typically scheduled at one site daily with flexibility to visit others as needed
Qualifications:
- Master of Science in Nursing (MSN)
- Current Nurse Practitioner license in North Carolina
- Basic Life Support (BLS) certification
- Prior experience in Long Term Care strongly preferred
Schedule:
- Monday through Friday, 8 AM to 5 PM
- No weekends or holiday shifts
