Job Description

A clinical role is available for a Nurse Practitioner (NP) specializing in long-term care, focused on providing comprehensive healthcare for patients residing in skilled nursing and assisted living facilities. This position supports patients by delivering hands-on, primary, preventative, urgent, and acute care directly at the facilities, helping to minimize unnecessary hospitalizations and promote smooth transitions to home settings.

Key responsibilities include developing individualized problem lists and medical treatment plans, stratifying high-risk patients for more frequent visits, and coordinating closely with primary care physicians and facility staff to ensure seamless care. You will perform health assessments regularlyat least monthly or more often when neededto monitor and improve patient health and prevent emergency room visits or hospital stays wherever possible. Additionally, this role involves performing single visits for patients transitioning into the facilities from hospital discharge plans, facilitating safe and healthy adjustments.

Schedule:

  • Monday through Friday
  • 8 AM to 5 PM
  • No weekends or holidays

Qualifications / Desired Experience:

  • Master of Science in Nursing (MSN) degree
  • Valid Maryland Nurse Practitioner License
  • Basic Life Support (BLS) certification
  • DEA license
  • Experience in Long Term Care settings preferred
