Job Description

This opportunity focuses on providing comprehensive care to patients in nursing home and assisted living settings. The program aims to treat patients in place when appropriate, helping to avoid unnecessary hospitalizations and supporting successful transitions to home environments. Monthly health assessments, or more frequent as needed, are performed to improve care and reduce emergency room visits or hospital stays.

Schedule:

  • Monday - Friday, 8 AM - 5 PM
  • No weekends or holidays

Key Responsibilities:

  • Develop and maintain a problem list and individualized medical treatment plans for each resident
  • Provide primary, preventative, urgent, and acute care on-site
  • Identify and stratify high-risk patients for more frequent clinical visits
  • Create contingency plans for high-risk health conditions
  • Collaborate closely with residents primary care physicians
  • Deliver hands-on bedside care, develop care plans, and coordinate with facility staff, including Directors of Nursing (DON)
  • Conduct single visits for patients newly discharged to the facilities to ensure safe, healthy transitions out of the skilled nursing facility (SNF)
  • Manage a panel of members across multiple nursing facilities, typically scheduling visits at one facility per day, with flexibility to visit others as needed for acute assessments

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • Current Maine Nurse Practitioner (NP) license
  • Basic Life Support (BLS) certification
  • Drug Enforcement Administration (DEA) registration
  • Experience in Long Term Care preferred
