Job Description
Providing comprehensive healthcare for residents in nursing home and assisted living settings, this role emphasizes in-place treatment to reduce unnecessary hospitalizations and supports smooth transitions back to home environments. You will conduct thorough health assessments on a monthly basis or more frequently as needed, ensuring tailored care plans that improve outcomes and reduce emergency interventions.
Schedule:
- Monday to Friday
- 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for each resident
- Deliver primary, preventative, urgent, and acute care directly on-site
- Identify and stratify high-risk patients, providing more frequent clinical visits as needed
- Create contingency plans for managing high-risk conditions
- Collaborate closely with residents primary care physicians and facility staff
- Provide hands-on bedside care, develop care plans, and coordinate with the Director of Nursing and other team members
- Conduct single visits for patients recently discharged from hospitals to nursing facilities, facilitating safe transitions
- Manage a panel of members across multiple facilities, typically covering one facility per day with potential visits to others for acute assessments
Qualifications:
- Master of Science in Nursing (MSN) degree
- Current New York State Nurse Practitioner license
- Basic Life Support (BLS) certification
- DEA license
- Experience in Long-Term Care settings preferred
