Job Description

Providing comprehensive healthcare for residents in nursing home and assisted living settings, this role emphasizes in-place treatment to reduce unnecessary hospitalizations and supports smooth transitions back to home environments. You will conduct thorough health assessments on a monthly basis or more frequently as needed, ensuring tailored care plans that improve outcomes and reduce emergency interventions.

Schedule:

  • Monday to Friday
  • 8 AM to 5 PM
  • No weekends or holidays

Key Responsibilities:

  • Develop individualized problem lists and medical treatment plans for each resident
  • Deliver primary, preventative, urgent, and acute care directly on-site
  • Identify and stratify high-risk patients, providing more frequent clinical visits as needed
  • Create contingency plans for managing high-risk conditions
  • Collaborate closely with residents primary care physicians and facility staff
  • Provide hands-on bedside care, develop care plans, and coordinate with the Director of Nursing and other team members
  • Conduct single visits for patients recently discharged from hospitals to nursing facilities, facilitating safe transitions
  • Manage a panel of members across multiple facilities, typically covering one facility per day with potential visits to others for acute assessments

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • Current New York State Nurse Practitioner license
  • Basic Life Support (BLS) certification
  • DEA license
  • Experience in Long-Term Care settings preferred
