Job Description
A rewarding opportunity is available for a Nurse Practitioner (NP) focused on providing high-quality care within nursing home and assisted living settings in New York, NY. This role centers on delivering comprehensive healthcare to insurance members in skilled nursing and assisted living facilities, aiming to treat patients in place when possible, reduce unnecessary hospitalizations, and support a smooth transition back to home environments.
Schedule:
- Monday through Friday, 8 AM 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop and maintain detailed problem lists and medical treatment plans for each resident
- Deliver primary, preventative, urgent, or acute care directly on-site
- Identify and stratify high-risk patients for more frequent clinical visits
- Formulate contingency plans addressing high-risk conditions
- Communicate and collaborate closely with residents primary care physicians
- Provide routine hands-on patient care, including bedside assessments, care plan development, order creation, and coordination with facility staff and leadership
- Conduct single visits to patients newly transitioned from hospitals to ensure safe and effective discharge plans
- Manage a panel of members across multiple nursing and assisted living facilities, typically scheduled at one location per day with flexibility to visit others as needed
Qualifications:
- Master of Science in Nursing (MSN) degree
- Valid New York Nurse Practitioner License
- Basic Life Support (BLS) certification
- DEA Certification
- Experience in Long-Term Care preferred
