Job Description
Provide essential healthcare services as a Nurse Practitioner conducting home health risk assessments for Medicare Advantage plan members in the Guernsey County area. This role offers the flexibility to create your own schedule, performing patient evaluations in various locations including Cambridge Township, Cambridge, and Jackson Township, OH. Each assessment typically takes 45 minutes and focuses on comprehensive review and education without requiring treatment planning or prescribing.
Responsibilities
- Conduct annual home health risk assessments for Medicare Advantage patients
- Review and evaluate past and current medical history, conditions, and medication regimens
- Educate patients on their medical conditions and medications to promote better health outcomes
- Identify gaps in care and provide actionable recommendations to close these gaps
- Perform assessments in patients homes within designated Guernsey County areas
Qualifications & Desired Experience
- Valid Nurse Practitioner License in Ohio
- Current Basic Life Support (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months current NP clinical experience preferred
- MSN degree from an accredited nursing program
Schedule & Location
- Per diem position offering flexible shifts any day Monday through Sunday
- Create your own schedule with a minimum commitment of weekly or bi-weekly hours
- Visit patients within Guernsey County, including Cambridge Township, Cambridge, and Jackson Township
