Job Description
A healthcare role is available in Mooresville, NC, focusing on providing specialized care for patients in nursing home and assisted living settings. This contract position offers the opportunity to deliver comprehensive care to insurance members in skilled nursing and assisted living facilities. The goal is to treat patients in place whenever appropriate, preventing unnecessary hospitalizations and supporting successful transitions back to the home setting.
Schedule:
- Monday to Friday
- 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for each resident.
- Provide primary, preventative, urgent, or acute care on-site.
- Identify and stratify high-risk patients to ensure more frequent clinical visits.
- Establish contingency plans for managing high-risk conditions.
- Collaborate and communicate effectively with residents primary care physicians and facility staff.
- Deliver hands-on bedside care, develop care plans, and create medical orders.
- Conduct assessments during patient transitions from hospital to facility and facilitate safe discharges.
- Manage a panel of members across multiple skilled nursing facilities, with daily scheduling typically at one facility and flexibility to visit others as needed.
Qualifications:
- Master of Science in Nursing (MSN) Degree.
- Basic Life Support (BLS) certification.
- Valid Nurse Practitioner License in North Carolina.
- Experience in Long Term Care is preferred.
