Job Description

A health-focused opportunity is available for a Nurse Practitioner committed to providing comprehensive care within nursing home and assisted living environments. This contract position supports insurance members by delivering in-place treatment to reduce unnecessary hospitalizations and facilitate smooth transitions back to home settings when appropriate. Regular health assessmentsat least monthlyensure ongoing patient needs are managed to prevent emergency room visits and hospital stays.

Schedule:

  • Monday to Friday, 8 AM 5 PM
  • No weekends or holidays

Key Responsibilities:

  • Develop problem lists and individualized medical treatment plans for residents
  • Deliver primary, preventive, urgent, and acute care onsite within nursing and assisted living facilities
  • Identify and monitor high-risk patients with more frequent clinical visits
  • Establish contingency plans for managing high-risk conditions
  • Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing
  • Provide bedside care, develop care plans, and create medical orders
  • Conduct single visits for patients transitioning from hospitals to facilities, assessing needs and supporting safe discharge plans
  • Manage a patient panel across multiple nursing facilities, scheduling care primarily at one location daily but flexible to cover urgent assessments elsewhere

Qualifications:

  • Master of Science in Nursing (MSN) Degree
  • Current Virginia Nurse Practitioner License
  • Basic Life Support (BLS) certification
  • DEA registration
  • Experience in Long Term Care preferred
