Job Description
Deliver comprehensive primary and acute care services for patients in nursing home and assisted living settings, focusing on reducing unnecessary hospitalizations and supporting successful transitions to home environments. This role requires providing skilled health assessments on a regular basis, managing high-risk patient care, and collaborating closely with primary care physicians and facility staff.
Responsibilities:
- Develop individualized problem lists and medical treatment plans for each resident.
- Provide primary, preventive, urgent, and acute care on-site at nursing and assisted living facilities.
- Stratify high-risk patients for more frequent clinical visits and develop contingency plans for their care.
- Collaborate effectively with residents primary care physicians and facility personnel, including Directors of Nursing.
- Perform routine bedside care, develop patient care plans, and create appropriate orders.
- Assess patients transitioning from hospital discharge plans to ensure safe recovery within the facility.
- Manage a panel of members across multiple facilities, typically scheduled at one facility per day but able to visit others as needed for acute care assessments.
Qualifications:
- Master of Science in Nursing (MSN) degree.
- Active Virginia Nurse Practitioner (NP) license.
- Basic Life Support (BLS) certification.
- DEA certification.
- Previous experience in Long Term Care settings is preferred.
Schedule:
- Monday through Friday, 8 AM to 5 PM.
- No weekend or holiday shifts.
