Job Description
A healthcare role focusing on comprehensive care within skilled nursing and assisted living settings offers the opportunity to make a meaningful impact on patient outcomes. This contract position based in Danbury, CT, involves delivering hands-on bedside care, developing individualized care plans, and collaborating closely with facility staff and primary care physicians to promote patient health and prevent unnecessary hospitalizations.
Schedule:
- Monday through Friday
- 8:00 AM to 5:00 PM
- No weekends or holidays
Key Responsibilities:
- Develop detailed problem lists and medical treatment plans for residents
- Provide primary, preventive, urgent, and acute care on-site
- Stratify high-risk patients to allow for more frequent clinical visits
- Develop contingency plans for high-risk conditions
- Collaborate effectively with residents primary care physicians and facility staff
- Conduct assessments for patients transitioning into the facility after hospital discharge, ensuring a safe recovery and transition back to the community
- Manage a panel of members across multiple facilities with flexibility to visit different locations as needed
Qualifications:
- Master of Science in Nursing (MSN) degree
- Current Connecticut Nurse Practitioner license
- Basic Life Support (BLS) certification
- DEA registration
- Experience in Long Term Care preferred
