Job Description

A Nurse Practitioner role is available in Cranston, RI, focusing on comprehensive care for patients in nursing home and assisted living settings. This position supports insurance members in skilled nursing and assisted living facilities by providing in-place treatment when appropriate. The goal is to minimize unnecessary hospitalizations and facilitate successful transitions back to home care. Monthly health assessments, or more frequent as needed, are conducted to enhance patient care and reduce emergency room visits and hospital admissions.

Schedule:

  • Monday to Friday, 8 AM - 5 PM
  • No weekends or holidays

Key Responsibilities:

  • Develop problem lists and medical treatment plans tailored to each residents needs
  • Provide primary, preventive, urgent, and acute care onsite
  • Identify and stratify high-risk patients for enhanced clinical attention
  • Design contingency plans for managing high-risk conditions effectively
  • Collaborate closely with residents primary care physicians and facility staff including Directors of Nursing (DON)
  • Deliver hands-on bedside care, create care plans, and make clinical orders
  • Conduct assessments for patients discharged from hospitals to ensure safe transitions out of skilled nursing facilities
  • Manage a panel of members across multiple nursing facilities, typically scheduled at one facility per day with flexibility for acute patient visits at others

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • Current BLS certification
  • Nurse Practitioner license valid in North Carolina
  • Experience in Long Term Care preferred
