Job Description
Providing comprehensive care in nursing home and assisted living settings, this Nurse Practitioner role focuses on delivering high-quality medical treatment to insurance members within skilled nursing and assisted living facilities. The position aims to treat patients on-site to prevent unnecessary hospitalizations and support successful transitions back to home environments. Regular health assessments are conducted at least monthly, with additional visits as needed, ensuring optimal care and timely intervention.
Schedule:
- Monday to Friday
- 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for each resident
- Provide primary, preventative, urgent, or acute care directly at the facilities
- Identify and stratify high-risk patients for more frequent clinical supervision
- Create contingency plans for patients with high-risk conditions
- Collaborate closely with residents primary care physicians
- Deliver hands-on bedside care, develop care plans, and coordinate with facility staff, including Directors of Nursing and primary care providers
- Conduct post-discharge visits to assess patients recently transitioned from hospital to facility, ensuring safe and effective care continuation
- Manage a panel of members across multiple nursing or assisted living facilities, with daily scheduling primarily at one facility and occasional visits to others for acute patient assessments
Qualifications:
- Master of Science in Nursing (MSN) degree
- Basic Life Support (BLS) certification
- Valid New York State Nurse Practitioner license
- DEA registration
- Experience in long-term care preferred
