A clinical opportunity is available for a Nurse Practitioner to deliver comprehensive care within nursing home and assisted living settings. This role focuses on providing high-quality, preventative, and acute care to insurance members residing in skilled nursing and assisted living facilities. The goal is to treat patients in place when appropriate, helping to avoid unnecessary hospitalizations and encourage successful transitions back to home settings.
Responsibilities include conducting regular health assessmentsat least monthly or more frequently as neededto monitor patient status and adjust care plans accordingly. The Nurse Practitioner will develop problem lists and individualized medical treatment plans for each resident and provide hands-on bedside care. Collaboration with primary care physicians, facility staff, and Directors of Nursing is essential to coordinate care effectively. Additionally, the role involves stratifying high-risk patients for more frequent clinical visits and creating contingency plans for complex conditions. Scheduled visits include single assessments of patients recently transitioned to the facility as part of their hospital discharge plans, ensuring smooth and safe transitions.
Qualifications for this position include a Master of Science in Nursing (MSN), New York State Nurse Practitioner license, DEA certification, and Basic Life Support (BLS) certification. Experience in long-term care is preferred to manage the distinct challenges of nursing home and assisted living environments.
Schedule:
- Monday through Friday
- 8 AM to 5 PM
- No weekends or holidays
