A rewarding opportunity is available for a Nurse Practitioner skilled in providing comprehensive care within nursing home and assisted living settings. This contract position focuses on delivering specialized healthcare to insurance members in skilled nursing and assisted living facilities. The goal is to provide in-place treatment when appropriate, helping to avoid unnecessary hospitalizations and support successful transitions back to home environments.
Patients receive thorough health assessments at least monthly, with more frequent evaluations as needed, ensuring the delivery of timely care and resources aimed at reducing emergency room visits and preventable hospital stays.
Location
This role is based in Brooklyn, NY, a vibrant borough known for its diverse communities, cultural landmarks, and excellent amenities. Brooklyn offers an enriching environment where urban living meets a strong sense of community.
Schedule
- Monday through Friday
- 8 AM to 5 PM
- No weekends or holiday work
Key Responsibilities
- Develop individualized problem lists and medical treatment plans for residents
- Provide primary, preventive, urgent, and acute care directly onsite
- Identify and stratify high-risk patients to increase clinical monitoring
- Create contingency plans tailored to high-risk conditions
- Communicate and collaborate with residents' primary care physicians and facility staff
- Deliver hands-on bedside care, develop care plans, issue orders, and coordinate with Directors of Nursing and primary physicians
- Conduct single visits for patients newly transitioned from hospitals to facilities to ensure a safe and healthy SNF-to-home transition
- Manage a panel of members across multiple facilities, typically attending to one facility daily with potential travel to others as needed
Qualifications
- Master of Science in Nursing (MSN) degree
- Valid New York Nurse Practitioner license
- Basic Life Support (BLS) certification
- DEA certification
- Experience in Long Term Care settings preferred
