Job Description

This opportunity focuses on providing comprehensive care to patients in nursing home and assisted living settings. The role centers around delivering in-place treatment to skilled nursing and assisted living facility residents, aiming to reduce unnecessary hospitalizations and promote successful transitions to home care. Regular health assessments are conducted at least monthly, with increased frequency as needed, to optimize healthcare outcomes and prevent emergency room visits when possible.

Schedule:

  • Monday to Friday
  • 8 AM to 5 PM
  • No weekends or holidays

Key Responsibilities:

  • Develop individualized problem lists and medical treatment plans for residents
  • Deliver primary, preventative, urgent, and acute care on-site
  • Stratify high-risk patients for increased clinical visits and monitoring
  • Establish contingency plans for managing high-risk conditions
  • Collaborate and communicate with residents primary care physicians and facility staff
  • Provide hands-on bedside care, coordinate care plans, and create medical orders within the nursing facility
  • Conduct individual visits for patients discharged from the hospital to assess readiness and ensure safe transitions
  • Manage a patient panel across multiple nursing facilities, with daily assignments to specific locations and flexibility to visit additional sites as needed for acute care

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • Current Maine Nurse Practitioner license
  • Basic Life Support (BLS) certification
  • DEA registration
  • Experience in long-term care preferred
