Job Description

Provide comprehensive, patient-centered care within skilled nursing and assisted living settings, focusing on minimizing unnecessary hospitalizations and promoting smooth transitions back to home environments. This contract position offers a Monday through Friday schedule, 8 AM to 5 PM, with no weekends or holiday shifts.

Key Responsibilities:

  • Develop individualized problem lists and medical treatment plans for each resident
  • Deliver primary, preventive, urgent, and acute care on-site
  • Identify and stratify high-risk patients for more frequent and focused clinical visits
  • Create contingency plans for managing high-risk conditions proactively
  • Collaborate effectively with residents primary care physicians and facility staff
  • Provide hands-on bedside care and develop thorough care plans in coordination with Directors of Nursing and other healthcare professionals
  • Conduct post-hospital discharge assessments in the facility to facilitate safe and healthy patient transitions
  • Manage care for a panel of members across multiple nursing facilities, typically scheduled at one facility per day with flexibility to visit others as needed

Qualifications:

  • Master of Science in Nursing (MSN) degree
  • Current Maryland Nurse Practitioner license
  • Basic Life Support (BLS) certification
  • DEA registration
  • Experience in long-term care preferred
