Job Description
Deliver comprehensive care as a Nurse Practitioner focused on patients in nursing home and assisted living settings. This contract role supports insurance members in skilled nursing and assisted living facilities by providing on-site assessments and care designed to promote health, prevent unnecessary hospitalizations, and facilitate smooth transitions to home when appropriate. Engage in monthly (or more frequent) health evaluations to address patient needs proactively and improve outcomes within these residential care environments.
Schedule:
- Monday through Friday
- 8:00 AM to 5:00 PM
- No weekends or holidays
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for each resident
- Provide primary, preventative, urgent, and acute care on-site
- Identify and stratify high-risk patients for more frequent clinical assessments
- Create contingency plans to manage high-risk conditions effectively
- Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing
- Deliver hands-on bedside care, create and update care plans, and issue medical orders
- Conduct single visits for patients transitioning from hospitals, ensuring safe discharge plans and recovery monitoring
- Manage a panel of members across multiple nursing facilities, providing care at one facility daily, with flexibility to visit others as needed for acute cases
Qualifications:
- Master of Science in Nursing (MSN) degree
- Current Nurse Practitioner license in New York
- Basic Life Support (BLS) certification
- DEA certification
- Experience in Long Term Care preferred
