Home Health LPN Opportunity in Memphis, TN

We’re looking for an experienced Licensed Practical Nurse (LPN) to join our Memphis home health team! If you have a strong background in home health care, experience with HomeCare HomeBase, or managing home-to-home visits, this role could be the ideal fit.

Position Highlights:

Direct Patient Care: Deliver compassionate nursing care, including medication management, wound care, and regular health assessments, directly in patients' homes.
Home Visit Management: Efficiently manage a home-to-home schedule to maintain consistency and high standards of care for each patient.
Detailed Documentation: Ensure patient records are accurate and up-to-date using HomeCare HomeBase to support comprehensive care.

Requirements:

Active LPN license in Tennessee.
At least one year of recent experience in home health (private duty experience not included).
Proficiency in HomeCare HomeBase or previous experience with home-to-home visit routines.
Current BLS certification from the American Heart Association.

Position Details:

Schedule: Full-time, 40 hours per week, Monday through Friday, 8:00 AM - 5:00 PM.
Mileage Reimbursement: Offered for travel within Memphis and surrounding areas.
