Job Title: Home Health LPN - 13-Week Contract
Location: Masontown, PA

Position Overview:
Sunbelt Staffing is hiring a skilled and compassionate Licensed Practical Nurse (LPN) for a 13-week home health contract in Masontown, PA. This opportunity is designed for an experienced LPN dedicated to delivering quality care in patients' homes.

Key Responsibilities:

Conduct patient assessments, administer medications, and provide wound care in a home health environment
Collaborate effectively with a multidisciplinary team to ensure comprehensive care plans
Document patient care and progress accurately using the Home Care Home Base system

Qualifications:

At least 1 year of recent home health experience
Proficiency in the Home Care Home Base charting system
Active LPN license in the state of Pennsylvania

Contract Details:

13-week assignment with the possibility of extension based on performance and staffing needs
