About Us: We are seeking a dedicated School Case Manager for a full-time position near Aurora, Colorado. Join our compassionate team and make a difference in the lives of K-12 students with mild to moderate disabilities during the 2023-2024 school year.

Responsibilities:

Serve as a liaison between students, families, teachers, and support staff.
Conduct assessments to identify students' needs and develop individualized education plans.
Monitor students' progress and provide necessary support and interventions.
Collaborate with educators to ensure a supportive and inclusive learning environment.
Advocate for students and ensure their educational and emotional needs are met.

Requirements:

Have at least two years of supervised experience in case management or a related field.
Experience or background in working with K-12 students with mild to moderate disabilities.
Bachelor's or Master's degree in Education, Psychology, or a related field (preferred).
