An experienced Home Health Registered Nurse (RN) role is available for a dedicated professional to provide skilled nursing care in patient homes within the Woodbridge, VA area. This full-time contract position offers the chance to impact patients' lives through comprehensive assessments, individualized care plans, and close collaboration with interdisciplinary teams. The role demands a self-motivated, organized nurse capable of delivering high-quality, patient-centered care independently.
Responsibilities include conducting detailed OASIS assessments, delivering skilled nursing services such as wound care, medication administration, IV therapy, and managing chronic conditions. The RN will monitor patient progress, educate patients and families, maintain accurate electronic documentation using Home Care Home Base (HCHB), and ensure compliance with regulatory standards. Coordination with therapists, social workers, and aides is essential for effective care delivery.
Key Qualifications:
- Active RN License in good standing for the state of practice
- Minimum 1 year of recent Home Health experience, with fieldwork preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS
- Strong clinical assessment and critical thinking abilities
- Reliable transportation with valid driver's license and insurance
- Excellent communication, time management, and organizational skills Preferred qualifications include a BSN, experience managing 2532 points weekly, and knowledge of Medicare and home health compliance.
Schedule and Work Details:
- Monday through Friday, 8:00 AM 5:00 PM
- Caseload of 2532 points per week (approximately 35 patient visits daily)
- Field-based care with daily travel within a designated territory, with mileage reimbursement provided
