Job Description
An experienced, compassionate Registered Nurse is needed to provide exceptional care to patients in their homes. This contract opportunity offers a chance to apply clinical expertise in a dynamic, patient-centered environment, ensuring quality outcomes and continuity of care for those most in need.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- 13-week contract assignment
- Field-based (direct patient care in home settings)
- Daily travel within designated territory (mileage reimbursed)
- Caseload: 2532 points per week (typically 35 visits per day)
Key Responsibilities:
- Conduct comprehensive OASIS assessments and develop individualized care plans.
- Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management.
- Monitor and evaluate patient progresspromptly communicating changes to healthcare teams and physicians.
- Provide clear education to patients and families on disease processes, treatment options, medications, and preventive measures.
- Ensure accurate documentation using Home Care Home Base (HCHB) and participate in care conferences and team meetings.
- Collaborate closely with interdisciplinary professionals, including therapists and aides, for holistic care.
- Adhere to clinical and regulatory documentation standards.
Qualifications:
- Active, unrestricted RN license in the state of practice.
- At least 1 year of recent home health nursing experience (field preferred).
- Proficiency in Home Care Home Base (HCHB) and OASIS required.
- Strong assessment, clinical judgment, and time management skills.
- Reliable transportation, drivers license, and valid auto insurance.
- Ability to work autonomously and adapt to changing priorities.
- BSN preferred and prior experience managing home health points caseloads highly valued.
- Knowledge of Medicare and home health regulations is a plus.
