Job Description

Providing compassionate end-of-life care in patient homes and residential settings, this Hospice Registered Nurse (RN) role involves managing comprehensive hospice care plans and collaborating with an interdisciplinary team to ensure patient comfort and dignity. This full-time, 13-week contract position requires strong clinical skills and empathy for patients facing terminal illness.

Schedule and Work Environment:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional weekend or on-call rotation as needed
  • Field-based visits to patient homes, assisted living facilities, or nursing homes
  • Caseload of approximately 45 patient visits per day
  • Daily travel within assigned service area with mileage reimbursement

Key Responsibilities:

  • Conduct thorough nursing assessments and develop individualized hospice care plans
  • Deliver skilled nursing focused on symptom and pain management to enhance patient comfort
  • Administer medications and treatments per physician orders and hospice protocols
  • Monitor patient conditions, communicate changes to the hospice medical director and care team
  • Educate patients and families on disease progression, medication administration, and support strategies
  • Collaborate with social workers, chaplains, aides, and volunteers to meet physical, emotional, and spiritual needs
  • Document care accurately using Home Care Home Base (HCHB)
  • Participate in case conferences, team meetings, and quality improvement initiatives
  • Ensure compliance with hospice policies and regulatory guidelines

Qualifications:

  • Active RN license in good standing in the state of practice
  • Minimum of 1 year of recent hospice nursing experience
  • Proficiency with Home Care Home Base (HCHB)
  • Strong clinical assessment and communication abilities
  • Reliable transportation, valid drivers license, and auto insurance
  • Compassionate, emotionally resilient, and able to work independently

Preferred:

  • BSN degree
  • Experience in palliative or end-of-life care settings
  • Knowledge of Medicare hospice regulations and documentation standards
