An experienced and compassionate Registered Nurse (RN) specializing in home health is sought to provide skilled nursing care to patients in their homes within the South Portland, ME area. This contract position offers a field-based opportunity to deliver patient-centered care that promotes safety, health, and independence.
Responsibilities include performing thorough OASIS assessments, developing and managing individualized care plans, and coordinating care with interdisciplinary teams. The role demands clinical proficiency in wound care, medication administration, IV therapy, and chronic disease management. Timely and accurate documentation using Home Care Home Base (HCHB) is essential. You will educate patients and families on treatments and preventive measures while monitoring patient progress and communicating any changes effectively to the healthcare team.
Key Responsibilities:
- Conduct comprehensive OASIS assessments; create and update individualized care plans
- Provide skilled nursing care including wound care, medication administration, IV therapy, and disease management
- Monitor patient status, communicate changes to physicians and interdisciplinary team members
- Educate patients and families on healthcare plans and preventive practices
- Maintain detailed, timely electronic documentation in Home Care Home Base (HCHB)
- Collaborate with physical therapists, occupational therapists, social workers, and home health aides
- Attend case conferences, staff meetings, and ongoing education sessions
- Comply with regulatory and agency standards
Qualifications:
- Active Registered Nurse license in good standing for the state of practice
- Minimum 1 year of recent home health field experience preferred
- Proficiency with Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment abilities and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance required
- Ability to work independently and maintain a flexible, field-based schedule
- Excellent communication, organizational, and time management skills Preferred:
- Bachelor of Science in Nursing (BSN)
- Experience managing 2532 points weekly (35 patient visits/day)
- Knowledge of Medicare and home health compliance protocols
Schedule & Contract Details:
- Full-time, 13-week contract
- Monday to Friday, 8:00 AM 5:00 PM
- Caseload averaging 2532 points weekly
- Mileage reimbursement for daily travel within assigned territory
