An exciting opportunity is available for an experienced and dedicated Registered Nurse specializing in Home Health care. This field-based role focuses on delivering comprehensive, patient-centered care to individuals in their own homes, emphasizing both skilled nursing and holistic support. Candidates must be passionate about improving patient outcomes independently while managing documentation and communication with a multidisciplinary team.
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- 13-week full-time contract
- Field-based: Daily travel required within the assigned territory, with mileage reimbursement provided
Qualifications:
- Active RN license in good standing
- At least 1 year of recent field-based Home Health experience
- Proficiency in Home Care Home Base (HCHB) software and OASIS assessments
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Able to manage a caseload of 2532 points per week (generally 35 patient visits daily)
- Excellent communication, time management, and organizational skills
- BSN and prior experience with Medicare compliance standards preferred
Key Responsibilities:
- Perform thorough OASIS assessments and create individualized care plans
- Provide skilled nursing care, including wound care, IV therapy, medication administration, and chronic disease management
- Educate patients and families about care plans, medications, and disease management
- Communicate patient status and progress to the broader care team and physicians
- Maintain accurate, timely records using HCHB
- Collaborate closely with PTs, OTs, social workers, and home health aides for coordinated care
- Participate in team meetings, case conferences, and ongoing education
