Providing compassionate end-of-life care as a Hospice Registered Nurse (RN) demands clinical expertise and a heartfelt commitment to patient dignity. This full-time, 13-week contract opportunity in Smoaks, SC, involves delivering hospice care to patients in their homes, assisted living, or nursing facilities within the service area.
Schedule and workload include Monday through Friday, 8:00 AM to 5:00 PM, with occasional weekend or on-call duties. A typical day consists of approximately 45 patient visits, requiring daily travel with mileage reimbursement provided. Documentation is completed electronically using Home Care Home Base (HCHB).
Key Responsibilities:
- Perform comprehensive nursing assessments to develop individualized hospice care plans.
- Provide skilled nursing focused on symptom management, pain control, and patient comfort.
- Administer medications and treatments per physician orders and hospice protocols.
- Monitor and promptly communicate changes in patient conditions to the care team.
- Educate patients, families, and caregivers on disease progression, comfort strategies, and medication management.
- Collaborate with interdisciplinary teamsincluding social workers, chaplains, and CNAsto address physical, emotional, and spiritual needs.
- Maintain timely, accurate documentation in HCHB.
- Participate in case conferences, team meetings, and quality improvement efforts.
- Ensure compliance with hospice policies, procedures, and regulations.
Qualifications and Experience:
- Active RN license in good standing for South Carolina.
- Minimum of 1 year recent hospice nursing experience.
- Proficient in Home Care Home Base (HCHB).
- Strong clinical assessment and communication skills.
- Knowledge of pain management, end-of-life care principles, and family support strategies.
- Reliable transportation, valid drivers license, and auto insurance.
- Ability to work independently in a field-based environment.
- Compassionate, emotionally resilient attitude. Preferred:
- BSN degree.
- Experience in palliative or end-of-life care.
- Familiarity with Medicare hospice regulations and documentation standards.
