Deliver expert nursing care as a Home Health Registered Nurse (RN) providing compassionate, patient-centered services in home settings. This full-time, 13-week contract role involves managing a caseload of 2532 points weekly with 35 patient visits per day, Monday through Friday from 8:00 AM to 5:00 PM. Key Responsibilities:

• Conduct comprehensive OASIS assessments and develop individualized care plans.
• Provide skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress and promptly communicate changes to physicians and interdisciplinary team members.
• Educate patients and families about treatment plans, medications, and preventive care.
• Maintain accurate and timely documentation using Home Care Home Base (HCHB).
• Collaborate effectively with physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and continuing education.
• Comply with regulatory standards and agency policies for documentation and care delivery.

Qualifications:

• Active RN license in good standing.
• Minimum 1 year recent home health experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessments required.
• Strong clinical assessment, critical thinking, and organizational skills.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work independently and manage a flexible field-based schedule.
• BSN preferred.
• Knowledge of Medicare and home health compliance standards is a plus.
