Job Description
Deliver compassionate end-of-life care as a Hospice Registered Nurse (RN) in a field-based setting, supporting patients and their families through this critical phase with dignity and clinical excellence. This full-time 13-week contract role offers the opportunity to directly impact patient comfort and quality of life, collaborating closely with a multidisciplinary hospice care team.
Location: Ruffin, SC a community known for its welcoming atmosphere and proximity to scenic outdoor spaces, offering a balanced lifestyle with access to a slower pace of living and local conveniences.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional weekend or on-call rotation as needed
Key Responsibilities:
- Perform comprehensive nursing assessments and develop individualized hospice care plans
- Provide skilled nursing focused on symptom relief, pain management, and patient comfort
- Administer medications and treatments following physician orders and hospice protocols
- Monitor patient conditions, communicating changes promptly to the medical director and care team
- Educate patients, families, and caregivers on comfort measures and disease progression
- Collaborate with social workers, chaplains, aides, and volunteers to address holistic care needs
- Accurately document all care activities using Home Care Home Base (HCHB) system
- Participate in team meetings, case conferences, and continuous quality improvement
- Ensure compliance with hospice policies and regulatory requirements
Qualifications and Experience:
- Active RN license in good standing for the practicing state
- Minimum one year of hospice nursing experience required
- Proficiency with Home Care Home Base (HCHB) electronic documentation
- Strong clinical assessment and communication skills, with a solid grasp of pain management and end-of-life care principles
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently managing a field-based schedule
- Compassionate, resilient demeanor required
- BSN preferred and familiarity with Medicare hospice regulations is a plus
