A healthcare organization is seeking a skilled Home Health Registered Nurse (RN) to provide compassionate, patient-centered nursing care in the Rockville, MD area. This field-based role involves managing comprehensive assessments, individualized care plans, and coordination with interdisciplinary teams to promote optimal health outcomes for patients receiving care at home.
Responsibilities include conducting OASIS assessments, delivering skilled nursing treatments such as wound care, medication administration, IV therapy, and chronic disease management, as well as educating patients and families on healthcare plans and disease prevention. The role requires accurate documentation using Home Care Home Base (HCHB) and active communication with healthcare providers regarding patient progress and changes in condition. Travel within the assigned territory to patient homes is essential, and mileage reimbursement is provided.
Qualifications:
- Active RN license in good standing for the state of practice
- Minimum one year of recent home health experience, with field experience preferred
- Proficiency in Home Care Home Base (HCHB) and OASIS assessments
- Strong clinical assessment and critical thinking skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to work independently with excellent organizational and communication skills
- BSN preferred
- Experience managing 2532 points per week and knowledge of Medicare and home health compliance standards are a plus
Job Details:
- Contract Type: Full-time, 13-week contract
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM
- Caseload: 2532 points weekly, typically 35 patient visits per day
- Location: Field-based, patient homes within Rockville, MD and surrounding area
