An opportunity is available for an experienced and compassionate Home Health Registered Nurse. This role is vital in delivering skilled nursing care directly to patients in their homes, ensuring seamless, patient-focused care within a supportive, interdisciplinary team. Schedule & Assignment Details:

• Monday to Friday, 8:00 AM 5:00 PM
• 13-week contract
• Caseload expectation: 2532 points per week (generally 35 patient visits daily)
• Field-based visits (travel within assigned territory; mileage reimbursement provided)

Core Responsibilities:

• Perform comprehensive patient assessments using OASIS and develop individualized care plans.
• Deliver skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient status, identify changes, and communicate promptly with care teams and physicians.
• Educate patients and caregivers on disease processes, care plans, medications, and health maintenance.
• Document care thoroughly and efficiently using Home Care Home Base (HCHB).
• Coordinate care with physical and occupational therapists, social workers, and home health aides.
• Participate in case conferences, team meetings, and ongoing education.
• Ensure all care delivery and documentation meets regulatory and agency standards.

Qualifications:

• Active RN license (in good standing)
• Minimum of one year of recent home health field experience
• Proficiency in HCHB and OASIS required
• Excellent clinical assessment, organizational, and communication skills
• Reliable transportation, valid drivers license, and auto insurance
• Ability to manage a flexible, autonomous schedule
• BSN, previous high-point caseload, and Medicare knowledge preferred
