An opportunity is available for an experienced and compassionate Home Health Registered Nurse. This role is vital in delivering skilled nursing care directly to patients in their homes, ensuring seamless, patient-focused care within a supportive, interdisciplinary team. Youll excel if you are self-driven, well-organized, and confident in both independent work and collaboration.
Schedule & Assignment Details:
- Monday to Friday, 8:00 AM 5:00 PM
- 13-week contract
- Caseload expectation: 2532 points per week (generally 35 patient visits daily)
- Field-based visits (travel within assigned territory; mileage reimbursement provided)
Core Responsibilities:
- Perform comprehensive patient assessments using OASIS and develop individualized care plans.
- Deliver skilled nursing care: wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient status, identify changes, and communicate promptly with care teams and physicians.
- Educate patients and caregivers on disease processes, care plans, medications, and health maintenance.
- Document care thoroughly and efficiently using Home Care Home Base (HCHB).
- Coordinate care with physical and occupational therapists, social workers, and home health aides.
- Participate in case conferences, team meetings, and ongoing education.
- Ensure all care delivery and documentation meets regulatory and agency standards.
Qualifications:
- Active RN license (in good standing)
- Minimum of one year of recent home health field experience
- Proficiency in HCHB and OASIS required
- Excellent clinical assessment, organizational, and communication skills
- Reliable transportation, valid drivers license, and auto insurance
- Ability to manage a flexible, autonomous schedule
- BSN, previous high-point caseload, and Medicare knowledge preferred
