An experienced Home Health Registered Nurse (RN) opportunity is available for a dedicated professional to deliver skilled nursing care in patients homes within Newnan, GA. This full-time, 13-week contract position focuses on providing high-quality, patient-centered care while working independently in the field. Responsibilities include conducting comprehensive OASIS assessments, developing individualized care plans, and providing skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management. You will monitor patient progress, educate patients and families, document care accurately using the Home Care Home Base (HCHB) system, and collaborate with a multidisciplinary team to ensure coordinated care. Participation in case conferences and adherence to all regulatory and agency standards is essential.
Qualifications and Desired Experience:
- Active RN license in good standing for the state of practice
- Minimum of 1 year recent home health experience, preferably field-based
- Proficiency with Home Care Home Base (HCHB) electronic documentation system
- Experience with OASIS assessment tool
- Strong clinical assessment and critical thinking abilities
- Reliable transportation, valid drivers license, and auto insurance
- Ability to manage a flexible, independent field schedule
- Excellent communication, time management, and organizational skills
- BSN preferred
- Prior experience managing 2532 patient points per week beneficial
- Knowledge of Medicare and home health compliance standards a plus
Schedule:
- Monday through Friday, 8:00 AM 5:00 PM
- Typical caseload: 2532 points weekly (35 patient visits per day)
- Field-based with daily travel to patient homes (mileage reimbursement provided)
