An opportunity is available for an experienced Home Health Registered Nurse (RN) to deliver exceptional skilled nursing care within patient homes. This role demands a self-motivated professional comfortable working independently while maintaining clinical excellence and strong patient communication.

The RN will conduct comprehensive patient assessments using OASIS, develop and manage individualized care plans, and collaborate with interdisciplinary healthcare team members to ensure optimal patient outcomes. Providing education to patients and families on disease management and preventive care is a key aspect, alongside timely and accurate documentation using Home Care Home Base (HCHB).

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop personalized care plans.
• Administer skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes, and communicate effectively with healthcare team and physicians.
• Educate patients and families regarding treatments, medications, and disease processes.
• Maintain accurate documentation within the HCHB electronic system.
• Coordinate care with physical therapists, occupational therapists, medical social workers, and home health aides.
• Participate in case conferences, staff meetings, and continuing education.
• Comply with all regulatory and agency standards for patient care and documentation.

Qualifications:

• Active Registered Nurse license in good standing in the state of practice.
• Minimum of 1 year of recent home health experience, preferably field-based.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment, critical thinking, and organizational skills.
• Reliable transportation with valid drivers license and auto insurance.
• Ability to work independently and manage a flexible schedule.

Preferred:

• Bachelor of Science in Nursing (BSN).
• Experience managing 2532 points weekly.
• Knowledge of Medicare and home health compliance standards.

Contract Details:

• Full-time, 13-week contract.
• Monday through Friday, 8:00 AM to 5:00 PM.
• Caseload of approximately 35 patient visits per day, totaling 2532 points weekly.
• Field-based role with daily travel reimbursed for mileage.
