Deliver impactful care directly to patients in their homes as a skilled Home Health Registered Nurse (RN) for a 13-week contract assignment. This full-time position offers the chance to make a meaningful difference by providing clinical expertise and compassionate support within a dynamic, patient-centered home health environment.

Schedule & Details:

• Full-time, contract duration: 13 weeks
• Monday Friday, 8:00 AM 5:00 PM
• Field-based role; daily travel to patient homes (mileage reimbursement provided)
• Caseload: 2532 points per week (typically 35 patient visits per day)
• Electronic documentation using Home Care Home Base (HCHB) and OASIS assessment tool

Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized patient care plans
• Deliver skilled nursing services: wound care, medication administration, IV therapy, chronic disease management
• Monitor and evaluate patient progress, identify changes, and communicate updates to care teams and physicians
• Educate patients and families on health conditions, medication regimens, and self-care techniques
• Maintain accurate and timely documentation in accordance with regulatory and agency requirements
• Collaborate with interdisciplinary teams, participate in case conferences and staff meetings, and engage in ongoing education

Qualifications:

• Active RN license in good standing
• At least one year of recent home health nursing experience (field experience strongly preferred)
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Demonstrated clinical assessment, organizational, and critical thinking skills
• Reliable transportation, valid drivers license, and current auto insurance
• Ability to work independently, managing a flexible field-based schedule

Preferred:

• BSN degree
• Experience managing a caseload of 2532 points per week
• Familiarity with Medicare and home health compliance standards
