An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide patient-centered skilled nursing care in home settings. This full-time contract role requires delivering quality nursing services, including comprehensive assessments, individualized care planning, and coordination with healthcare teams to promote positive health outcomes. Schedule:

• Monday to Friday
• 8:00 AM 5:00 PM

Responsibilities:

• Conduct comprehensive OASIS assessments and develop personalized care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, recognize changes, and communicate effectively with care teams and physicians.
• Educate patients and families on treatment plans, medications, and preventive care strategies.
• Maintain accurate documentation using Home Care Home Base (HCHB) system.
• Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
• Participate in case conferences, staff meetings, and ongoing education.
• Comply with all regulatory and agency documentation and care standards.

Qualifications:

• Active RN license in good standing for the state of practice.
• Minimum of one year recent home health experience; field experience preferred.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment, critical thinking, and independent working skills.
• Reliable transportation, valid drivers license, and auto insurance.
• Excellent communication, time management, and organization.

Preferred:

• BSN degree.
• Experience managing a caseload of 2532 points per week.
• Familiarity with Medicare and home health compliance standards.
