An experienced and compassionate Home Health Registered Nurse (RN) is needed to provide patient-centered skilled nursing care in home settings. This full-time contract role requires delivering quality nursing services, including comprehensive assessments, individualized care planning, and coordination with healthcare teams to promote positive health outcomes. Ideal candidates are self-motivated, organized, and able to work independently while maintaining clinical excellence and effective patient communication.

Schedule:
- Monday to Friday
- 8:00 AM 5:00 PM
Responsibilities:
- Conduct comprehensive OASIS assessments and develop personalized care plans.
- Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
- Monitor patient progress, recognize changes, and communicate effectively with care teams and physicians.
- Educate patients and families on treatment plans, medications, and preventive care strategies.
- Maintain accurate documentation using Home Care Home Base (HCHB) system.
- Collaborate with therapists, social workers, and home health aides to ensure coordinated care.
- Participate in case conferences, staff meetings, and ongoing education.
- Comply with all regulatory and agency documentation and care standards.
Qualifications:
- Active RN license in good standing for the state of practice.
- Minimum of one year recent home health experience; field experience preferred.
- Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
- Strong clinical assessment, critical thinking, and independent working skills.
- Reliable transportation, valid drivers license, and auto insurance.
- Excellent communication, time management, and organization.
Preferred:
- BSN degree.
- Experience managing a caseload of 2532 points per week.
- Familiarity with Medicare and home health compliance standards.
