A dedicated Home Health Registered Nurse (RN) opportunity is available for experienced professionals ready to deliver skilled nursing care in patients homes. This role requires independent work with clinical excellence and strong patient communication to ensure high-quality, patient-centered care.

Key Responsibilities: Perform comprehensive assessments using the OASIS tool and develop individualized care plans. Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management. Monitor patient progress, recognize changes, and communicate with healthcare teams and physicians promptly. Educate patients and families on conditions, treatments, medications, and preventive care. Maintain accurate, timely documentation in Home Care Home Base (HCHB). Collaborate with interdisciplinary team members such as physical therapists, occupational therapists, social workers, and home health aides. Participate in case conferences, staff meetings, and ongoing education. Ensure compliance with regulatory and agency standards.

Qualifications: Active RN license in good standing within the state. Minimum one year recent home health experience; field experience strongly preferred. Proficiency in Home Care Home Base (HCHB) and OASIS assessments. Strong clinical assessment, critical thinking, organizational, and communication skills. Reliable transportation, valid drivers license, and auto insurance. Ability to work independently with a flexible, field-based schedule. Preferred: BSN degree. Experience managing 2532 points weekly. Familiarity with Medicare and home health compliance.

Schedule & Location: Full-time 13-week contract. Monday through Friday, 8:00 AM 5:00 PM. Field-based with daily travel to patient homes within the assigned territory; mileage reimbursement provided.
